2024 Required Forms
New & Returning Rowers
Before returning to water practices this fall, each rower must submit all their required forms, a valid Virginia High School League Physical and verify that they have an active basic individual membership with USRowing.
Required Forms
Please print each these of forms, complete them and email them as a pdf to pacrew.web@gmail.com. Please provide your name in the subject line and ensure that each form is separate from the others (i.e. do not allow one form to begin on the back page of the previous form). Each form must be printed and put into a separate file. Thank you!
VHSL Physical
Please download THIS PHYSICAL FORM - you are responsible for completing part of it and some of it must be completed by a doctor. When complete, email it no later than August 28th to pacrew.web@gmail.com. It must be reviewed by our Assistant Coach before you can row on the water. Failure to submit by this date my disqualify you from water practice until it is approved by the Assistant Coach.
USRowing Membership
Each rower must have a valid basic individual membership with USRowing before they can participate in on the water practices. If you are a returning rower and need to renew your USRowing membership follow the instructions in THIS DOCUMENT.
