Required Forms for New & Returning Rowers
All new and returning rowers must complete the following before they may return to water practices with PACC:
1. The following packet of forms must be completed and given to the Club secretary. You may either print them and bring them to PACC's Fall Information Night (date tbd), OR email the forms to the following address: firstname.lastname@example.org. If you choose to email your pdf, PLEASE make sure that EACH document is separate from the others - they must be broken up into files by the type of document, so make sure each one is separate in your pdf when you email it back. Thank you!
2. You must also submit a valid physical from the Virginia High School League. You must email it to email@example.com at least 1 week before Fall Season practices begin, Thursday September 9, 2021. We need sufficient time to review it for any missing information. Failure to do so may result in missing water practice.
3. You must also have a valid basic Individual US Rowing membership. For security reasons, information about how to do this will be posted on Band in the week prior to the start of the Fall Season.
